Title: Case Manager
Reports to: Program Director
Job Summary: The Prenatal Care Coordinator is responsible for meeting with pregnant women to determine and assess whether their situations qualifies for Prenatal Care Services in Wisconsin. We also strive to connect all pregnant women with community resources, medical providers, and finding a pediatrician for the baby. The screening assessments will occur at our office location or in the home of the potential client. Based on the outcome of the assessment clients will be deemed approved or not approved to receive prenatal care services through our company and all community resources and services will be made available to them. This role is responsible for successfully implementing the Prenatal Care Coordination model, goals and desired outcomes.
Job Duties/Responsibility:
· Meet with clients in their home or our office to complete potential client screenings and assessments.
· Identify needs, problems, and resources. Develop & complete care plan.
· Assist clients in developing a safe & healthy environment for themselves & their children
· Attend and actively participate in departmental meetings and trainings
· Complete special projects as assigned, community events such as baby showers, back to school events, parties, etc.
· Complete and maintain all required paperwork, records, documents, etc.
· Follow and comply with all safety and work rules and regulations. Maintain office housekeeping standard.
· Perform other duties as assigned by Program Director
Knowledge, Skills and Abilities:
· Ability to communicate effectively and interact with a diverse population
· Case notes and documentation skills
· Strong verbal and written communications skills as well as time management skills
· Organizational Skills
· Computer skills including Outlook, Microsoft Office. Ability to utilize internet and complete accurate data entry.
Experience/Education:
· High school diploma or equivalent (required)
· Two years of experience coordinating care for at-risk or low-income women (preferred)
Work Orientation Factors:
· While performing the duties of this job, the employee is regularly required to stand, walk, talk & hear. The employee frequently is required to sit.
· Job task may require the ability to lift, bend and move items from one place to another.
· The physical demands described here are representative of those that must be must by an employee to successfully perform the essential functions of this job.
Must have a valid Wisconsin driver’s license, have an acceptable driving record, carry minimum auto insurance as required by state law and provide own transportation.
