To qualify for PNCC/CCC services the state of Wisconsin requires you to be Medicaid/Badgercare eligible. Please bring your Forward Health Card to your appointments, we will need it for your file. If you don't have your card we can assist you with ordering a new card.
Clients who are enrolled in Prenatal Care Service must be pregant at the time of signing up. Proof of pregnancy is needed, ultrasounds are not accepted per state requirements. Proof needs to be a pregnancy statement from doctor or clinic.
If you have already given birth to your baby we can still help you. If your baby is 60 days or less you could qualify for our Child Care Coordination program. We assist parents in their children with making sure all appointments are up to date, employment opportunities, resources, etc.
This program serves Milwaukee County residents.
Clients are not allowed to work with multiple agencies at the same time. You can only work with one company at a time. If you ever want to stop working with us for any reason please let us know, and we will close your case.
