Prenatal Care Coordination (PNCC)
Prenatal Care Coordination includes all of the following:
- Outreach
- Initial Assessment
- Care Plan Development
- Ongoing Care Coordination
- Health Education & Nutrition Counseling (for clients with an identified need)
Child Care Coordination (CCC)
Our services don't just end once your baby is born. Our CCC program provides ongoing community resources and referrals after your baby is born. These referrals and resources consist of childcare, employment, clothing, housing, food, & more.
